The Nursing Home Transition Coordinator is Ann H. Neimer.  The Nursing Home Transition Program offers nursing home residents the alternative to live at home. It might include the referral to Home and Community Based Services for funding or one-time expenses such as security deposit; moving expenses, furniture, etc. Many persons are not aware of the support services available in the community.  Assistance may include locating housing; moving; set up of household; referrals to Home and Community Based Services; attendance at care plan meetings; Budgeting and Information and Referral.
