The Live Well 360 (LW 360) is a comprehensive person-driven assessment tool that will address 28 areas of a person’s life to evaluate areas that may need more skills training or identify a need to be filled. The areas are personal and life impacting such as housing, transportation, attendant care services, durable medical equipment, assistive technology, home modification, etc. The LW 360 will create a Live Well Plan that provides a road map that not only focuses on physical health, but also includes these other areas of quality of life activities. Each will be individualized to the consumers’ unique needs, mapping the person with a disability's way to greater independence and opportunity to live well. A LW 360 is particularly useful when a person is ready to be discharged from a hospital or rehabilitation facility. There are often times when a person living with a disability will run into road blocks when attempting to access products, goods and services. Through advocacy, the CILCP's experienced staff may be able to work along side of you to remove these roadblocks.
Nursing Home Transitioning (NHT) Services consist of an array of services which support Consumers' goals to transition fom an insitutional setting to an independent living setting within the community. NHT will assist anyone wanting to relocate from a nursing home back into community living by identifying and overcoming barriers to moving from the nursing facility to the community. Some of these barriers include housing, rental assistance, household item, and transportation from the facility and in the community. CILCP's NHT program serves the following counties: Adams, Bedford, Blair, Cumberland, Franklin, Fulton, Huntingdon, Juniata, Mifflin and Perry Counties. Many people are unaware of the many resources that are available to them within their communities. CILCP's experience staff can assist persons with disabilities with navaging through the maze of confusion of fragmented programs and services that will enable them to live well with their disabilities. The CILCP's experienced staff have accumulated knowlege of many years of community resources and specialize in working with people with diverse disabilities.

Wheels In Motion is a grassroot coalition composed o persons with diverse disabilities. Primarily funded by a grant from the Office of Vocational Rehabilitation and is part of a coordinated statewide advocacy efforted supported by the PA State Independent Living Council. Wheels In Motion will be advocating for positive change within Community Based Services and accessible affordable transportation within their local communities. Wheels meet the first Wednesday of each month at 1 pm in the CILCP Community Room.
IL Skills Training classes offer individuals the opportunity to learn and practice the skills needed to live independently in the community. These trainings can be provided in small groups or on an individual basis.
