Independent Surveys

The IM4Q (Independent Monitoring for Quality) program is contracted with 3 Administrative Entities totaling 6 counties in Central PA to conduct quality of life surveys for individuals with Intellectual Disabilities.  Those counties include Cumberland, Perry, Dauphin, Huntingdon, Mifflin, and Juniata.  IM4Q conduct 290 surveys.  The surveys are conducted in a team of two, by individuals with intellectual and other disabilities, family members of individuals with disabilities and others interested in becoming an independent monitor/contractor with the program. 


The surveys are conducted at the convenience of the individual being surveyed and can be completed at their home, place of employment, day program or any place of their choosing.  Information collected on the survey tool goes back to the state Office of Developmental Programs who overseas any and all services for the Intellectual Disability population.  The data is used in reports for future planning, baseline and ongoing compliance measurements, along with considerations that can result.  These considerations are areas of change requested by the individual, their family, staff or whomever is involved in the survey process.
